CONTINUING EDUCATION - DENTISTS, HYGIENISTS and ASSISTANTS
All Texas licensed dentists and dental hygienists are required to complete 12 hours annually of continuing education in accordance with the Dental Practice Act. Omega Seminars is offering the opportunity to earn a total of 6 hours of accredited continuing education in two, half day seminars while cruising aboard Carnival's Triumph November 1 to 5, 2012. Robert G. Williams, D.D.S., D-ABFO will present all seminars.

SPEAKER;
Dr. Robert Williams is a 1977 graduate of Baylor College of Dentistry and has been in private practice for 35 years. He is currently the Chief Forensic Odontologist for the Dallas, Webb and Jefferson County Medical Examiners' Office. He is adjunct clinical professor for Baylor College of Dentistry, and the UNT Health Science System in Ft. Worth, research associate for the center of Human Identification, Dept. of Forensic Anthropology at the University of North Texas, and guest lecturer for the Southern Methodist University Dept. of Anthropology. He has been a consultant to the Texas Rangers, Federal Bureau of Investigation, Texas Attorney General's office, U.S. Attorney's Office, Drug Enforcement Administration, and the Judge Advocate General's Office of the U.S. Dept. of the Navy and currently serves with the Dept. of Health and Human Services Region VI of D-MORT, where he was deployed to the World Trade Center in 2001, and the aftermath of Hurricane Katrina in 2005 and Hurricane Ike in 2008 for the recovery of human remains. He is a founding member of the Dept. of Homeland Security, a member of the American Society of Forensic Odontology, Fellow of the American Academy of Forensic Sciences, and a Diplomate of the American Board of Forensic Odontology where he served six years on the Board of Directors and as Chairman of the Ethics, Articles and By Laws, and Nominating Committees. He is also a member of the American Dental Association, Texas Dental Association, and the Dallas County Dental Society where he is the designated expert witness and has served on the peer review and judicial review committees. He has been with the Southwestern Institute of Forensic Sciences for the past 20 years where he has testified in more than 85 criminal trials in the State of Texas and Louisiana, and has done more than 4,000 dental identifications and bite mark analysis.

PART #1- Friday morning seminar begins at 9:00AM: With the recent popularity of all the television shows on forensic science, forensic odontology has become a popular subject with dental health care professionals. This course will cover briefly all aspects of forensic odontology including dental identification, standard of care, bite mark analysis, recognition of human abuse, the dentist's role in mass disasters, and the dentist's / hygienists critical role in maintaining accurate records to be used as evidence in legal proceedings. The CSI effect will be discussed in an attempt to separate fact from fiction of what one sees on television as to what is reality in the world of forensics. WARNING: DUE TO THE NATURE OF THIS SUBJECT, CAUTION IS ADVISED IF GRAPHIC SCENES OF HOMICIDAL VIOLENCE ARE OFFENSIVE OR DISTURBING TO THOSE WISHING TO PARTICIPATE.

PART #2- Sunday morning seminar begins at 9:00AM: Although Part #2 is a continuation of the PART #1 course, you may take only PART #2 for 3 CEU's.

CONTINUING EDUCATION CREDITS: (Total of 6 CEU's): These courses have been approved by the Texas Dental Hygienists' Association and State Board of Dental Examiners approved provider of continuing education for DENTISTS, DENTAL HYGIENISTS, AND DENTAL ASSISTANTS.


ITINERARY:

*Thursday, November 1, 2012
arrive in Galveston, Texas between Noon and 2:00PM for embarkation on Carnival's Triumph. You must be checked-in and aboard the Triumph by 3:00PM. If you are driving, parking in a secured lot is available for $10 per day. The Triumph sails at 4:00PM from Galveston to Cozumel, Mexico.
*Friday, November 2, 2012
your first seminar (#1) at sea: sign in at 8:30AM, seminar begins at 9:00AM and runs until 12Noon. The balance of the day is free time to enjoy your self, network, and make new friends.
*Saturday, November 3, 2012
we arrive in Cozumel, Mexico at 9:00AM at which time you may go ashore and enjoy the beauty of this carefree island. You must be onboard the Triumph by 4:00PM. Sailing is at 5:00PM. Missed sailings are participant's responsibility to arrange transportation.
*Sunday, November 4, 2012
your second seminar (#2) at sea: sign in at 8:30AM, seminar begins at 9:00AM and runs until 12NOON. The balance of the day is free time.
*Monday, November 5, 2012
we arrive back in Galveston, Texas for debarkation beginning around 8:00AM.

ON A FIRST COME BASIS AND BEFORE JUNE 30, 2012 A COMPLETED CRUISE / SEMINAR APPLICATION FORM ALONG WITH YOUR REGISTRATION DEPOSIT OF $150 PER PERSON IS DUE TO CONFIRM YOUR CABIN and SEMINAR RESERVATION. If you are using a credit card, you will see the charge on your statement as Carnival Cruise Lines. If you are paying by check, please make your check payable to PARADISE VACATION AND TRAVEL CO. A monthly installment payment plan is available. Full and final payment is due by AUGUST 31, 2012. If you are paying by credit card, your payments will automatically be charged to your credit card on file when due.

PENALTIES AND CANCELLATIONS:
For the group rate before August 31, 2012 no penalty for cancellation, August 31, 2012 and before September 15, 2012 a penalty of $150 per person for cancellations, September 15, 2012 and before October 1, 2012 a penalty of 50% of your cruise / seminar cost is assessed, October 1, 2012 and before October 15, 2012 a penalty of 75% of your cruise / seminar cost is assessed and as of October 15, 2012, no refund for cancellations or missed sailings. Penalties apply to all guests in a cabin and are assessed in the event of either a partial or full cabin cancellation. Substantial penalties for one name change per cabin under the group rate.

ALL CANCELLATIONS AND CHANGES MUST BE IN WRITING AND RECEIVED BY PARADISE VACATION AND TRAVEL CO. to be effective. It is the traveler's responsibility to have a current valid PASSPORT or other acceptable proof of citizenship required for sailing (re-entry into the United States) and to arrive at the respective ports on time for scheduled sailings. If you miss a sailing, it is your responsibility to arrange passage to the next port. Carnival Cruise Lines and others reserve the right to refuse passage to anyone deemed a safety risk. Please advise Paradise Vacation and Travel Co. with a detailed note attached to the application form if you have any medical condition including pregnancy that both Carnival Cruise Lines and PVT should be aware of. This includes the use of wheel chairs, motorized scooters, and medications needing refrigeration and or injecting.
The cost of your cruise includes your cabin accommodations, all meals, entertainment, current government taxes, port charges, and security fees. Not included is your transportation to Galveston, parking fees, the standard $11.50 per person per day gratuity for your service personnel aboard the Triumph, alcoholic and carbonated beverages, insurance, shore excursions, possible fuel surcharge, and items of a personal nature.
