CONTINUING EDUCATION – DENTISTS, HYGIENISTS and ASSISTANTS
All Texas licensed dentists and dental hygienists are required to complete 12 hours annually of continuing education in accordance with the Dental Practice Act. Omega Dental Seminars is offering the opportunity to earn a total of 6 hours of accredited continuing education in two, half day seminars while cruising aboard Carnival's Triumph November 3 to 7, 2011. Chris Norton, DDS will present all seminars.

 

SPEAKER;

Dr. Chris Norton got his Bachelor's degree in Chemistry from Southern Methodist University and his DDS degree from Baylor in Dallas, Texas. He was in private practice from graduation to 1996 when he became Director of the Dental Programs at Amarillo College. He then joined a group practice in 2001 where he was the top producer for the largest orthodontic provider in Utah. He came back to Texas in 2007 and established his current practice, the Fast Trax Orthodontics in McKinney, Texas.

Dr. Norton, a member of many professional organizations including the ADA, TDA, and the AGD, is a dynamic, motivating speaker; one who will get you excited about the new innovations and directions dentistry has to offer.


COURSES:

#1-"AIRWAY TO HEAVEN": A look at how important the airway is when considering the proliferation of 21st century diseases"
(Friday morning seminar) We will review major points from previous lectures on "How to Grow Attractive Faces" including how the airway affects beauty. Now we will turn to see how the airway affects health. We will look into the possible links between airway and ADHA, Sleep Apnea, Asthma, Allergies, Chronic Fatigue Syndrome, Anxiety/Panic Attacks, Dementia, and others.

#2-"TREATING THE AIRWAY": An introduction to Buteyko Breathing Method and Orofacial Myology" (Sunday morning seminar) We will be introduced to Buteyko and Orofacial Myology including principals of both treatments modalities. Several exercises from both methods will be explored. In addition, we will look at how the Myofunctional Trainers can be used in conjunction with these treatments.

CONTINUING EDUCATION CREDITS: (Total of 6 CEU's): These courses have been approved by the Texas Dental Hygienists' Association a State Board of Dental Examiners approved provider of continuing education for DENTISTS, DENTAL HYGIENISTS, AND DENTAL ASSISTANTS.

arrive in Galveston, Texas between Noon and 2:00PM for embarkation on Carnival's Triumph. You must be checked-in and aboard the Triumph by 3:00PM. If you are driving, parking in a secured lot is available for $10 per day. The Triumph sails at 4:00PM from Galveston to Cozumel, Mexico.

your first seminar (#1) at sea: sign in at 8:30AM, seminar begins at 9:00AM and runs until 12Noon. The balance of the day is free time to enjoy your self, network, and make new friends.
*Saturday, November 5, 2011
we arrive in Cozumel, Mexico at 9:00AM at which time you may go ashore and enjoy the beauty of this carefree island. You must be onboard the Triumph by 4:00PM. Sailing is at 5:00PM. Missed sailings are participant's responsibility to arrange transportation.
*Sunday, November 6, 2011
your second seminar (#2) at sea: sign in at 7:30AM, seminar begins at 8:00AM and runs until 11:00AM. The balance of the day is free time.
*Monday, November 7, 2011
we arrive back in Galveston, Texas for debarkation beginning around 8:00AM.

ON A FIRST COME BASIS AND BEFORE JUNE 30, 2011 A COMPLETED CRUISE / SEMINAR APPLICATION FORM ALONG WITH YOUR REGISTRATION DEPOSIT OF $150 PER PERSON IS DUE TO CONFIRM YOUR CABIN and SEMINAR RESERVATION. If you are using a credit card, you will see the charge on your statement as Carnival Cruise Lines. If you are paying by check, please make your check payable to PARADISE VACATION AND TRAVEL CO. A monthly installment payment plan is available. Full and final payment is due by AUGUST 31, 2011. If you are paying by credit card, your payments will automatically be charged to your credit card on file when due.

PENALTIES AND CANCELLATIONS:
For the Early Saver Rate both deposits and full payment is totally non-refundable, no name changes, no cabin changes, no category upgrades. For the group rate before August 31, 2011 no penalty for cancellation, August 31, 2011 and before September 15, 2011 a penalty of $150 per person for cancellations, September 15, 2011 and before October 1, 2011 a penalty of 50% of your cruise / seminar cost is assessed, October 1, 2011 and before October 15, 2011 a penalty of 75% of your cruise / seminar cost is assessed and as of October 15, 2011,
no refund for cancellations or missed sailings. Penalties apply to all guests in a cabin and are assessed in the event of either a partial or full cabin cancellation. Substantial penalties for one name change per cabin under the group rate.

ALL CANCELLATIONS AND CHANGES MUST BE IN WRITING AND RECEIVED BY PARADISE VACATION AND TRAVEL CO. to be effective. It is the traveler's responsibility to have a current valid PASSPORT or other acceptable proof of citizenship required for sailing (re-entry into the United States) and to arrive at the respective ports on time for scheduled sailings. If you miss a sailing, it is your responsibility to arrange passage to the next port. Carnival Cruise Lines and others reserve the right to refuse passage to anyone deemed a safety risk.

Please advise Paradise Vacation and Travel Co. with a detailed note attached to the application form if you have any medical condition including pregnancy that both Carnival Cruise Lines and PVT should be aware of. This included the use of wheel chairs, motorized scooters, and medications needing refrigeration and or injecting.

EVERYONE MUST HAVE A VALID PASSPORT FOR THIS CRUISE
The cost of your cruise includes your cabin accommodations, all meals, entertainment, current government taxes, port charges, and security fees. Not included is your transportation to Galveston, parking fees, the standard $10 per person per day gratuity for your service personnel aboard the Triumph, alcoholic and carbonated beverages, insurance, shore excursions, possible fuel surcharge, and items of a personal nature.
